During the Prologue (Monastery bit), instead of going down the well, continue on the path to the village. You'll start a quest named "Woe to the Vanquished". First you'll have to fight a soldiers (and save innocents) then you'll have to enter a house to stop soldiers from robbing villagers. Simply select the "Get out of here or die" dialogue to get the achievement.
