I just got this from playing the first level, no grenades (LB) and no ranged weapon (B). Not sure if grenades count as ranged attacks.
Throwing a weapon you've picked up is fine though, like a bat or cleaver.

Also tried this the very first time I played it, but gave up on the first boss, just couldn't beat him. Second time I tried it - after beating the game and playing a lot of survival - he was cake.
