To Achieve 100% you need to complete the following:
Complete the Undead Nightmare Campaign DLC
Save All Towns
Achieve Rank 5 in the four Undead Challenges
Complete every side missions (The Green Circle with the Question Mark in it)
Find and Break the Four Horses of the Apocalypse:
War - Located around Gaptooth Ridge to Marfarlene's ranch
Famine - Located around Chuparosa and Tesoro Azul
Pestilence - Located around Tall Tree
Death - Gaining by completeing the Campaign and can be found wondering around Mexico
Collect all outfit (There's three to unlock)
And Find and Break the Mythical Unicorn

I think you need to add to your list that you need 74 survivors left alive in the towns to achieve 100% completion. I completed everything you listed above and I didn't get the achievement.
After all of the frustration, I have only come up with one theory as to why it didn't get it, I didn't have 74 survivors. I was across the map in Mexico on a mission when a town got over run. I entered a safe house to save my game and transport to the town. Immediately after I saved my game a person died in Gapped Tooth Ridge. At this point I have no way of saving that person. I reloaded my save and used my horse to travel to the town but he dies before I get there. I tried using travel, but I can't beat the clock and he dies when I spawn in the map.

So after completion My STATS show I have 73, not 74, survivors and 96% completion on the game! No more objectives, no more challenges, nothing left to do and I am a small percent off!!! That one surviver that died ruined my 100% completion achievement. I thought I was getting everything, but I guess not.

It is not true you need to have 74 survivors alive. I myself just attained 100% with only 72 survivors alive according to the stats.

I posted that the survivors in the towns prevented my achievement from unlocking. Anthirian #6 is right, it does not make a difference for the completion achievement. I have received the %100 with lower numbers.
The real issue was that I completed the story without finding all 4 horses. I was given "death" after the story was complete, so I didn't think I had to find it. BUT you have to find it on your own. I can't really say for sure how it this happened, but I at random found "death" up in tall trees and when I captured it the achievement unlocked.

@Bobalue Same thing with me I just got zombied and says only 4 and there doesnt seem to be any other missions available anywhere else.
@aFluxx My stats say I have 78 survivors... I dont know what that means but its more than the 74 everyone is mentioning.

Do you have to do all the missing persons one right after the other? I did the first one and decided to ignore the second for awhile to do other missions, but when I rode off the icon just disappeared. I don't want to progress the story too much just to find out I need to reload from right after saving the first missing person.
So do I have to do all the Fort Mercer ones right now? Or will the poster come back eventually?

@Chaos_Wolf it doesn't mean Dr. Zed from Borderlands at all. :) It's a quote from Quentin Tarantino's "Pulp Fiction" when Butch (Bruce Willis) has stolen a police officer (Zed)'s chopper. His girlfriend asks him who Zed is and he replies "Zed's Dead, baby."
As for this achievement, I don't have the zombie hunter outfit as I've already been zombied. If I do all the requirements will it still unlock? And I also have 79 survivors left, just to add the the amount of evidence against the number of survivors argument...

you have to keep 74 survivors alive only one time, i think. maaybe when you get
the "spinning plates" achievement

Ok all. At the top it says that "Death" is located in Mexico. Some people say that
they have found it in Tall Trees. Is there a place that is easier to find the
horse? Do you have to be on a regular horse to run into it? Please help!!

Do weapons count? Because apparently the Semi-auto shotgun is easily missable - it's in the very last mission. Also, the high power pistol you apparently get of the doctor(randomly appearing character) - but moments before i read that, i shot the doctor in the head. Is this going to affect my chances?
I had undead nightmare up to 90% then stupidly deleted my saves... Found the challenges horendously easy this time though -appart from the one where you need to set fire to bears.

@#20 I can confirm, at least, that you do not need to be on a regular horse to get Death, as I was riding Pestilence when I found him. Seems to recall Death was around Perdido (East) in Mexico.

I can confirm that as long as you save all survrs and terrortories one this is all you need to help towards 100%. i had only 22 survrs left and still got the cheevo

guyz i do really need help about this mission it says that i saved 65 surv And i completed 4 mission and cancelled 12 and i finished 95 % and i did every thinggg
what should i do to get 100 %? there is really nothing to do

just check everything, use the social club website, takes a few seconds to set up, once the checklist appears you should be well aware of what you have done/not done, it took me 7 hours to finish it, thats what it said on the stats, mind you i rushed most of it, and did a little free roam, it was by far the hardest achievement on the game, along with the other 100%, good luck, i hope you can get it.

If 5-10 hours of added story/ gameplay is worth $20, then yeah. It's also a stand alone disc you could rent off gamefly though too.

To all the people talking about it, Death can be found ANYWHERE on the map. He seems to show up for most people in Mexico but if you read he has no set location/locations. Name a place on the map and you can probably find someone who found Death around that area.
Death is annoying because you just have to look, but the good thing is because he can show up anywhere you can kind of ignore it and stumble upon him. I searched for about 30 mins got annoyed and gave up. Was on my way to a mission and he just randomly appeared.

The 1st girl you save from the Fort Mercer Missing People, does that count towards the 16? Because Rockstarsocialclub still has not registered i saved her. I went to Mexico and saved 2 from El Presidio and those registered right away.

The outfits thing is confusing. I just got this achievement and it was a lot of fun, undead nightmare is the best. For some reason, UN lists outfits in the stats which are not relevant to UN specifically. #5 is right that you don't need to unlock all of the outfits for 100% on UN because most of them are from regular old RDR. #43 those are the only outfits available to zombie marston. If you have unlocked them you've done everything outfit related in UN, although it seems the union suit is part of the DLC too.

I can verify that you don't have to find Death in the wild. Also, tomahawk challenges aren't required for 100% completion or for the challenge achievement.
