easiest way of getting this achievement is by completing the game a few times beforehand so that you and other squad mates all have spectre X weapons and if your lucky collossus armour. it doesnt really matter which squad mates you take with you but obviously the best if you're a soldier are garrus and wrex for combat and biotic/tech. if your a biotic then garrus and ashley and if your a tech, ashley and wrex. simple.
the hardest parts of the game are when you're in the mako because it sucks major balls. the best strategy is to hide the mako beyond the enemies and get out and snipe them (usually with high explosive rounds X due to killing any geth trooper in 2 hits)
bear in mind the mako takes damage when ur not in it and if u get in you just die.

Caution when going for the difficulty chievos. what should have been a 3 play run has turned into a 5 playthrough, as sometimes, hardcore mode does Not unlock.
A couple of my online buddies reported the same thing, and its happened to me and i definately went >start menu>difficulty change. start game. check, yep thats right, and still didnt unlock. just a heads up.

You can change the setting. You just have to do it as soon as you gain control of you character on the Normandy at the beginning of the game.

By far the easiest way of getting this is as a Soldier. When you have Immunity from getting your fitness up, and have full Shotgun skill, it's VERY easy. Just run in and pop Immunity, then blast the shit out of all your enemies. You'll barely take any damage.
As for using the Mako, you get less XP that way, so I used it just for moving around. It's not really that needed, even on Insanity. Most things that do significant damage are very easy to avoid, like Thresher Maws, rockets, etc...

Cant wait to check this out on insanity just started mass effect to due loads of great comments about the game normally its not my thing but so far so good expect im dying a lot on easy lol i think ill improve on my second playthrough interesting game im enjoying the achievements are a bit sucky really too many bloody playthroughs could have been a lot more interesting games shouldent require more than 2 playthrough to get 100% achievements in my opinion dont care how good a game is after 30 hours you will wana put it down for a while

once level 60 you can just run through story based missions only ignoring all side ones..
It is slighty annoying to have so many playthroughs. Just doing my third now on casual to reach lvl 60 and get Krogan/Quarian and all the decisions ready for number 2.. then will give insanity a try

If you install Frictionless Material WIII in your Assault Rifle you are literally able to shoot without stopping during a fight and it also give you extra damage. It should help you out of any situation.

i did another playthru to get veteran/hardened achvs and it didnt unlock. im not sure why they are so damn picky on how you change the difficulty. i was planning on importing my first character to make it easier, but wasnt sure if switching the difficulty on normandy would work. didnt want to take the chance so i started a brand new guy and put difficulty veteran. of course i finish the story and it doesnt unlock.

I was looking at some of the advice here and would like to give some of my own related to this cheevo:
1) I know that someone mentioned the Mako being the hardest part of Insanity...Yes and No. Yes, it is...If you stop and bother to shoot things. If you don't bother to stop and instead just plow through the enemies without slowing down (and this does take some bit of skill with the vehicle since it handles like a drunk walrus driving an elephant through 10 foot-deep mud), then no, it isn't that hard. I did this in all parts of the game and only had trouble in one spot (on Virmire I got hung up and one of the colossus things took me down).
2) Your guiding principle should be: Don't care. Those rachni babies in the hot lab? Don't care. Finding the serum for the people dying on Noveria?

Sorry, don't care. Is Commander Shepard lonely? Nope, don't care. Do only what you need to do to finish each area and screw all the rest. By doing this, you should have a 3-4 hour play-through. On level 60 it is really, really easy to breeze through this.
3) Since I played as an adept (I already saw soldier advice above): Do crowd control and this will be easy. Use lift and singularity to never allow big nasty things like Krogan Battlemasters to hit you and you will never, ever have trouble. I died three times in total on this playthrough. Also, as a side note: Saren is immune to singularity on this level. He is NOT immune to lift.
4) One final note: On Noveria I got stuck because I was trying to use Ashley and Wrex since they are big brawlers. Once you hit the door with the rachni o

I'm an engineer/tech expert and I easily beat the game on insanity and completed all of the side missions and collection missions on insanity. I went through the game on hardcore with my new tech expert and didn't do any side missions and ended the game on level 44. Then I started a new game on insanity with my level 44 tech expert.
The enemies were really easy to kill. I just send my two squadmates up a little bit ahead and I just blast away with my pistol and the enemies are a breeze. Master overload brings down almost all shields, then I just blast them with my pistol or snipe them (sniping was the bonus skill I chose). Most enemies I kill with my pisotl, I only use my squadmates when there is a big group of enemies.

You can beat the game on insanity easily with any character at any level if you just take cover, constantly upgrade all of your crap, strategically use your squadmates to your advantage, constantly use your powers, and manually level up everybody in a way that matches your fighting style.
With bosses, just hold RB and pound them with all of your and your squadmates' powers, then take cover and shoot until your powers recharge. Also hold RB to bring up the power wheel during combat so you can aim your weapon anywhere you want while combat is paused. It is an easy way to kill those annoying geth hoppers.

@1 everything you need to kill with the Mako, dies 10x faster by hand. Just use Tungsten of Schredder ammo on an assaultrifle and you're truly unstoppable for the entire game.
No kidding, this is seriously one of the easiest games on the hardest difficulty 0.o

I think Sentinel is the easiest class to beat this on. I beat the game once on hardcore with sentinel then reimported him into insanity and kicked absolute butt. My shield were able to withstand everything expect direct head hits from rockets...

Wasn't going to do this, but finished my second playthrough at level 57, so I'm gonna do this at the same time I work to get to level 60. Soldier, experienced with Garrus and Wrex, and I plan on doing the bare minimum to get through the game. Wish me luck.

@25 No, you MUST start the difficulty at Insanity before landing on Eden Prime, so the very first time you are able to move, pause, go to options, go to gameplay, and change the difficulty to the hardest one you have, because if it isn't Insanity, it'll be Hardcore. Unless it's your first playthrough, then, you should start off on a easy difficulty.
Also, the best way is absolutely Soldier w/ Immunity. This way, you are versatile with your weapons, and it is extremely hard to die. You would have to attempt to kill yourself to die with Immunity. And get Colossus armor from the C-Sec Requisition officer. If you have the HMW guns, you'll run through this in a few hours. For the HMWA put Frictionless Materials X on both upgrades, and Sledgehammer X rounds. This way, you never overheat, and

I use HMWA X, but with Frictionless Materials X, Scram Rail X, and Inferno Rounds X. Only overheats if you have a lead trigger finger, and can take out a Geth Prime in like 5 seconds.

@ #39 you haven't completed hardcore yet so insanity wont be unlocked, start a new game with an old profile (that way you keep all your guns/armour) when you start on the ship (before Eden prime) press start, gameplay, then select hardcore, scroll to the bottom of the page and click save and exit (like it says "hardcore/insanity is not veteran")

you can change the difficulty when the game starts and you're on the normandy. before you go to the captain and speak with nihlus, go to the menu and change the difficulty to insanity. (note: i believe you first have to complete a playthrough on hardcore) when you speak to nihlus, the captain will enter and you will eventually start the game on eden prime. the difficulty must be on insanity once you're on eden prime, or it does not count.

#44 as a soldier you should have access to great weapons and armor if you start the insanity playthrough with a previous character...you should already be level 60 before you start insanity with all the best weapons already in your aresnal...make sure all your crew are maxed out as well and they all have the best weapons...it is tough and you will die quite a few times, but stick with it and it is a very rewarding achievement!

Play Insanity with the Adept Bastion. Master statis and you should be at level 60 before trying this. The Bastion is the ONLY character that can freeze enemies and still hurt them while they are frozen.

This should be done BEFORE you talk to Nihlus and start the Eden Prime mission. After you land on Eden Prime, the last time to change difficulty is right before you investigate the Prothean Beacon. After that if its not set to Insanity, you can`t unlock the achievement. I changed it right on the Normandy after the conversation with Joker and Kaiden before you go to see the admiral. Hope that helps. Make sure to get the Krogan Battlemaster armor and equip it on Shepard as soon as you get it.

You`re welcome # 50. I hope that helped. Use the armor glitch to equip ALL the squad members with the better armors as you can put non human armor on humans and vice versa with that glitch. Also there is the glitch right before you help Liara break out of the bubble that she is trapped in. That would help boost up your exp considerably. I went from like level 3 or so all the way up to 46. All in the same spot and it was wel worth the time invested to level up that much on Insanity difficulty. Hope that helps also.

***UPDATE*** I am going to retract using the Battlemaster armor. While playing on Insanity, i ended up equipping the Krogan Colossus X armor on Shepard and found it to be the overall best armor for Shepard. If you come across other Colossus armor, i highly suggest holding on to them and equipping it to the 2 squad members you plan on using a lot. I hope that helps anyone who reads this.

The best way to get all the achievements as quickly as possible and get all the class specific achievements, (with the least amount of downtime) AND get the Long Service Medals is the following:
1st Playthrough: Normal Difficulty. Recommended class: Soldier. Ashley and Kaiden as Allies. Play through the game paying specific attention to the codex achievement and developing a romance. You'll earn the majority of the achievements on your first play through. I earned 30 my first time through. Use all your weapons, focus on having as much shields as you can (For Tactician). And max out your charm/intimidate and paragon/renegade score. This will be your main character you'll carry throughout the series and future playthroughs.

Special Playthroughs: Normal Difficulty. Recommended Class: Adept/Engineer. Choose your bonus ability as Adrenalin Rush (that you earned from your time with Ashley the previous playthrough). Any allies (it doesn't matter). Level up as quickly as possible. Most people recommend level 6 but the higher you can get the better because you can spend more points on skills the decrease the cooldown on your abilities.
Then for the Adept spam the Transfer terminals and Engineer spam the Mako. Using Adrenalin rush to reset your cooldowns and making this even quicker. No joke I got to level 8 with both classes and all the specialist achievements in one night about 3-4 hours watching YouTube the whole time
:-). Once you finish with the adept restart as an engineer and do it again. or Vice Versa

2nd Playthrough: Veteran Difficulty. Use same character as 1st Playthrough. If you have Barrier as a bonus ability I recommend getting it but Adrenalin Rush works well too. Use two allies you haven't used before. Max out Paragon or Renegade that you didn't do the first time and you'll probably get a 50th level character if not a 60th level character by the time your done. This will make your last playthrough much easier. I recommend getting both the Bring Down the Sky DLC as well as Pinnacle Station. I got all of my Ally achievements well before Ilos because I played through both DLCs and they popped I thought pretty early.

You got a perfect game on one the best series ever released. With only minimal amount of spamming/grinding for achievements. Hope this helps!

A good way to get some good stuff is to go to Intai'sai after completing Pinnacle Station. Save next to the computer. Then use the computer (220000 credits worth). If it something you want save, if it is something you don't want reload the save. The item you get is random so you don't waste any money.

I beat the game multiple times to get to a high level and got the specter gear with frictionless material on most of them so it basically never overheats. It was pretty easy for me.

If you have already played through a few times and this is the final trophy, there is one well documented XP glitch when saving Liara. It also helps if you are trying to save her first when on Insanity and can't beat the Krogan. All the details are on this GameFAQs thread.

Completed this yesterday. Overall, it was fairly simple, since I imported a previous character (default Shepard) and had Colossus X & Spectre X gear for everyone (Wrex & Garrus as allies). I occasionally had trouble with tech/biotic enemies.
Have to say organic enemies were ridiculously strong, defensive wise. My hat's off to anyone who can do this starting with a new character.

I Played Hardcore with an Engineer (Squadmates were Tali+Liara) and it was a breeze. I'm going to try this same tactic on Insanity, but with Tali's ability AI Hack, Liara's Crowd Control Ability's, and My Neural Shock, Hardcore wasn't even a challenge.
I'm hoping it's the same for Insanity. (I do plan on boosting Credits by doing a quick Causual Run-Through B4 I play Insanity though. Also, I started this character at level 1)
I really think this build is good for Insanity.

When I chose my difficulty setting it says that hardcore needs to be unlocked, but it's able to be chosen. Does that mean I need to play through on veteran first, or will the achievement pop for hardcore if I play through?

no it means it is unlocked. once i finished hardcore mode, i had insanity mode saying the exact same thing and i then chose it on the normandy before landing on eden prime and it worked. make sure you lock your change on the bottom line.
this difficulty is a piece of cake at lvl 60 with Kaiden and Liara as your partners and you as soldier. I mean at the end, Saren wasn't even able to take a shot at me since he was floating around (lift i suppose)all the time and all i had to do is stand there and shoot. same when he transform into sovereign and it was basically the same throughout the whole game.

@#68 I don't believe side assignments are necessary for completing the achievement. If you need a little more XP for leveling up, they might be useful, but I don't think they're required.

I love that everyone is giving wildly different team/ability combinations and touting them as best. This is a great game that rewards personalization and diverse styles of play. If you've played it the requisite two times to unlock Insanity, then you know and care enough to be able to beat it the way you want . Don't neglect the role-playing aspect for the sake of convenience. I viewed my first few playthroughs as achievement collection/familiarization, and then, when I really knew the game well, I played through on Insanity, making every choice carefully for the narrative's sake. Now I have the perfect character to import to ME2.
