You need to unlock the Mirror Match Combat Set first at the arena.
Hint: All enemies in this set have shield and armor only.

Bugs264 found a real easy way to get the achievement.
Just put the difficulty on the easiest setting and play all the way to the end of round 3. When there is one enemy left, change the difficulty back to normal or higher. Achievement unlocked!!!

You can't really camp against the mirror enemies. They throw grenades like crazy and always flank you. I'm not sure if #3 is legit, but I guess it's worth a try. I did it on Normal myself with the increased player damage on the Brimstone map.
The Brimstone map is probably the smallest of all of them, but I preferred it. Better for kills streaks and quicker games.

I've beaten all 3 games on Insanity with and without maxed gear, and managed to come out on top. But this on Insanity?... Would be a hell of a fight.
Still plenty fun.

This is insane. I have beaten everything Mass Effect could dish out on the Insanity level, and most of the levels are still fine to completed save a few like anything Collector related. I have every achievement available...
But I cannot beat this even on normal level...I mean my god I cannot do it. I can get to wave 3 but I get swarmed with all the Incinerates and wraps, once that happens its all over as Sheppard keeps falling forward so I can't move. Plus for the first time ever the squad does nothing...until their is only 1 enemy left. Both Wrex and Grunt just stood their and only bothered to swarm the last enemy.
I'll try out #3's trick. I'll do it quickly encase Bioware might try to patch it. I will not stand not having every achivement of any of the Mass Effect games.

Agree with behellmorph on the fact that squadmates do nothing most of the match. I had Wrex and Zaeed. Zaeed would usually pop a few shots, but Wrex stood still the whole time.
Tried it twice; gotten further each time. Trying it again in a bit. Let's see how it goes.
My advice would be focus on one "class" at a time. The sooner you eliminate one completely, the better your odds.

I do not see how you could possibly beat this on Insanity without being a max level Infiltrator with a Black Widow and lvl 5+ concentration mod. They flank way to fast, being able to slow things down with a sniper rifle helps was the only way I could do it on Insane. I have trouble on my other classes with it though. Glad to of finally gotten a challenge for once though. This DLC was very enjoyable, way better than Omega was.

The legit way on Insanity!
This is how I did it (For the Soldier):
10% Damage Boost
Squad Members: EDI (Incinerate) and Tali (Energy Drain: Maximum Damage and Recharge Speed)
I don't know if the drones and decoys did any good, but it can't hurt. :)
I used the M-7 Lancer X with Damage Mod V and Armor Pen Mod V. Hadne-Kedar Set for Max Weapon damage. Delumcore Overlay.
Focus on ONE clone at a time. Start with an energy to get rid of the shields and just go to town while throwing in Incinerate from EDI. Run around if need be and dodge any incoming fire. Use cover often. Change up your strategy for each round as the spawns will likely switch. Rinse and repeat and success should be yours.

I'm a vangaurd and this was pretty difficult even on normal. Mainly i just made sure to bring along james and javik for their grenades. I used the black widow sniper and i just ran a circle around the map. Toggle using the three shots of your sniper and then hit the enemies with the grenades as fast as you can. They should drop relatively fast, just keep trying and you'll get it.
Just got 100% again on this game again!

This doesn't need to be patched @ behellmorph. Normally, Wrex and Grunt bum rush everything and krogan charge/shotgun them to death, but for this match, you need to actually direct your teammates to attack specific enemies using up on the D Pad, because any teammate will play extra passively; it's meant to be more challenging. Also, make sure to use their grenades over and over while paused with the power wheel. Their massive amounts of health and barriers/shields combined with Fort/Barrier should keep them alive indefinitely while they charge in.
I made the mistake of going in there the first time with almost all the debuffs on; that just wasn't going to happen. I did it on the Armax Arsenal map because you can run and get to cover in many places and be safe from flanks. First try was

I did this on Brimstone with Garrus and Wrex on Normal. It's true that you have to tell your squad mates what to do, otherwise they'll stand and stare at you just like mine did while I was getting demolished. I focused on one clone at a time (as best as possible), using Garrus's Overload to drop the shield (one shot did it) and Wrex's Carnage to reduce the armor (didn't do much, I probably didn't spec it well for power). Incendiary ammo with just about any gun finished the job. In my case, I was hugely helped by having biotic charge fill up my shields every time I used it.
How it played out: there's a high wall at the far side of Brimstone (enemies sometimes spawn from there - it's the lower ground, not the platform). I spent a lot of time there (especially wave 3) shooting from behind

In the first case, I'd just biotic charge right back, immediately overload/carnage, and shoot the guy. 50%-60% of the time this one sequence killed the clone. Moving laterally behind cover was enough to avoid the brunt of any grenade blasts. Also, I only carried one weapon so my powers would recharge quickly (so I could use biotic charge almost immediately to recharge my shield; be careful where you point it - a few times I found my shield recharged but WAY out of cover and surrounded by clones. It ended poorly). When things got dicey, I ran across the map up the platform and set up shop there. You can get flanked more easily there but moving back and forth is hugely helpful. When you run straight to one end, the clones follow (pretty slowly) in a fairly straight line, which makes it much easier to line up Overloads, Carnage shots, etc. This also helps to isolate them as inevitably, one clone will arrive first. Good luck!

its not that all squadmates play "passively" in the mirror match, its that some characters are "broken"... wrex and grunt for example, so dont use them.
for any adepts looking to do it on insanity (assuming max levels on character and weapons):
- take garrus & james both with fully upgraded ammo powers (make sure you spec james' incendiary ammo for EB at lvl6)
- give them both a Particle Rifle, take the most powerful weapon you can but dont sacrifice your 200% weight bonus
- choose armor for either power damage or recharge for serious spamming
- bonus power = energy drain... put all powers to lvl6, leaving shockwave (if you havent already)
the ammo powers (ESPECIALLY JAMES' EXPLOSIVE BURST) do serious amounts of damage with the particle rifles (a brute on insanity drops in a co

- carnage + warp/throw combo for armor
- save james' grenades for when an enemy is in your face and your team is still cooling
- always remember to focus teammate fire with up on dpad for each enemy
- with a 1 sec cooldown on throw, remember to detonate your warps with it at every opportunity
- ive found the train map to be best suited to this strategy as its harder for them to flank you and easier to keep your team close together

As much as I love Garrus, he dies instantly on Insanity, and James won't last much longer either. Grenades are the thing you should rely on your teammates for(which Garrus doesn't have). Pain Train is a flank/get trapped in a corner fest, and as an adept, against these guys, combo detonations aren't doing much; if you say otherwise, you're lying about even trying this on insanity as an adept. Your lift grenades, plus your teammates'(preferably people with a ton of health) grenades, and running around a nice big map with lots of cover to replenish those grenades is a fairly easy solution.

And you mentioned something about brutes...there are no brutes here, and comparing a brute to one of these guys is pointless. For anyone who hasn't tried this yet, you're in for a real surprise in terms in of difficulty; these clone Shepards are TENACIOUS; they're in your face constantly, rushing, spamming powers, vanguards are charging and grabbing you, Soldiers using concussion shot, engineers with incinerate, etc. They have a ton of shields/barriers and armor, so they quickly overwhelm you, which is why grenades for their instant damage and no cooldown are the way to go, and on a bigger map, for the most part. Sorry but the usual methods just aren't as effective. And all characters seem to be broken, not just Wrex and Grunt, however they will respond to your commands.

I did this on Insanity with my Sentinel using James and Garrus on the glitch map and had no problems. If you set yourself up on any of the four corners opposite spawn behind the large block and set your team just ahead on the right and left you can pick apart each opponent. Prioritize Vanguards and Engineers and stay in hard cover you should get it first try through

Legit way in Insane:
Pick the original Armax map. It's the biggest one and it makes it easy for you to hide or run away.
Pick James and Javik and equip them with the particle gun. They both have grenades also. And best of all, they actually shoot at your clones without ordering them to. Whereas other characters are broken in this mode ( the krogans).
When the match starts, stay as far as possible from the action. Spam your teammates grenades on the clones and replenish it with ammo crates. Shoot and use your power on the clones but stay far away enough not to die.
Easy. I couldn't past the first wave in other maps, and I got on the first try on this one.

Hate to be the bearer of bad news for you guys who completed the Mirror Match method, but there is another, considerably easier, method.
After trying for 2 hours on Mirror Match (I'm playing in insanity mode and don't believe in altering the difficulty, EVER), I decided to try something different.
Reapers, super elite, armax original map, 3 handcaps: extra enemy damage, no medi-gel, no ammo drops.
I got between 11 and 12,000 points.
--Infiltrator, lvl 60
--Liara and Kaidan with Warp and Cryo Blast maxed, respectively
--Viper sniper rifle with time dilation and enhanced scope mods
--Tempest SMG with piercing mod and extended barrel
--used combo of cryo ammo with emphasis on armor weakening in tandem with incinerate with emphasis on extra damage
