Not sure why but I got both this and the A Dark Promise achievement in one playthrough.
I had cleared my hard drive cache and disconnected from xbox live in order to try to get the blight queller achievement which didn't seem to be working. Got what I was after, finished the game and lived. Reconnected to xbox live and when it updated, this achievement popped.

The achievement popped for me yesterday, for no reason too. I loaded a save with which I didn't play since March 2010. This was an auto-save just after the end of the game, I did the Dark Ritual in this playthrough.

Same happened to me. Hadn't played in over a year. Loaded an old save and it popped as well.

Same as #2. Did the Dark Ritual and then loaded the auto-save (Epilogue) and "Ding!" Achievenent unlocked. Still have to do all 3 other endings anyway for "Perfectionist".

What happens after Ultimate Sacrifice? You character dies and End Game?
Or is your character still playable somehow after it? Or can you ress him?

After sacrificing yourself the game ends similar to the other endings apart from instead of you talking to everyone in the throne room, what they do after origins is just explained like morrigans fate.

If you got funky with Alistair, you probably do NOT want to bring him on your final run on the Arch Demon (i.e. leave him behind at the gates of Denerim). He's pretty dead set on you living and him dying... I couldn't find anything to say to him to let me do the sacrifice.

This achievement popped for me today even though I let Morrigan do the ritual. After I reloaded the game for the purpose of finishing off the dlc, it popped.

#9 - same thing happened for me as well. So in that case does the game think you've done that ending for the purposes of "Perfectionist"?

I got sour news for you jack, just get to the witch's hunt dlc, hopefully after you played through all the other dlcs, b/c she will say he you didn't knock me up if you did. Even though I did, but I got this achievement for free only to find out I needed to replay from near the end of the game on.

Player was a Girl, Alisteir by my side. Romance with Alisteir.

I have been renting this from GFly for month and finished today with the Dark Ritual Ending. I got back to the title screen, reloaded the autosave and ding, got this one too.

I got this one as well for loading the Auto-Save after beating the last boss. Not complaining or nothing, I just find it rather amusing xD
