#1 is wrong,
i finished story on normal level, all riddles, trophies, Arkham spirit mystery, everything except challenges and story on hard level and i didn't unlock this achievement.
I think it happened because during this mission, i killed a henchman.
He was tied upside down, and i threw a batarang. He fell and died in toxin.
Okay, i know, Batman never would do that, but it was funny haha
Now i will do it on hard level to unlock it. |

Well, i didn't kill the henchman I just didn't save one guard.
Now I noticed you need to save two guards and one henchman to unlock it.
When i played it first time, i just saved one guard and one henchman. You will be able to continue thru the game without saving all of them, so this achievement can be missed! |
