Night Terrors not unlocking

Has anyone else had this problem? Finally got to the end of the campaign, no achievement. Realised i hadnt done the Scorchers on Ultra Nightmare so did that, no achievement.

The only other thing i hadnt done was Durars challenge, so i went and did that, no achievement.

Am i missing something? Or is it glitchy?

Questions:

Did you change the difficulty at any time, even for a second?
Did you do it all on one save file?
Did you go from start to finish in a campaign?
Did you use scenario selection?

If the answers are no, yes, yes, no, then I don't know what to say other than do it all again. Sorry.

Well i dunno what i did wrong but done it again now and it unlocked
