A couple raiders screwed me over after I had gone on a nice spree which set me back roughly half an hour on Fallout 3, it was about 5 a.m so I rage quit so I can go back to it....Now.

I was playing Special Forces: Team X and my frame rate dropped dramatically and cost me my life. I was dead before the frame rate returned to normal.

Last died playing Tranzit on Black Ops 2 zombies. Round 31 I think. I was running around in the town and got hit a few times, then the fire and lava finished me off.

On the final battle of chapter 14 on Spec Ops The Line. I actually died about 10+ times and was only playing on Suicide difficulty so not looking forward to FUBAR..those turrets do my nut in.

So, I'm playing Dead Space 2 on Hard Core, and experience some difficulty since I last played it in Summer. I make it until the subway cart at the end of Chapter 2, and remember how there'll be this scene where you dangle from your leg...

...when suddenly the door from the cart ahead of me comes off and crushes my head like a melon.

Playing sparten ops on legendary and my buddy was hanging back snipping while I was in close hitting them with shotgun and plasma grenades.

It was working good, he would take a shot on the Guardians and I would take out the knights.

Then he ran out of ammo and ran to reload. In that small ammount of time the knights spawned more guardians and overwhelmed me.

I drowned in Limbo. It took me a few tries to figure out the puzzle and then when I figured it out and reached the top, I accidently let the egg drop in the water...so I had to do it all over again.
