Achievement Not Unlocking

I beat the game this morning, and the green tab appeared saying "achievement unlocked" for Chaos In Disarray" but it never unlocked on my profile. When I look at the in game achievements, it is listed there, but it is missing from my gamercard. Any one else have this problem? I tried rebeating Chaos, but that didn't fix it.
