No Rest For The Living Secret Level - Level 9: March of the Demons

For anyone interested the last level for No Rest For The Living is a secret level which can be reached on Level 4: Hell Mountain. From what I've seen on the leaderboard you don't have to complete the secret level to get the achievement & avatar award.

How do you get there then??

HERE's HOW:

Near the end there will be a place where two Barons of Hell come out from two doors. Across from this there is a wall that you can use, press use here and then on the right wall, kill an Imp, then press the switch and a gate opens, go up this and go up an elevator, then run to the other side of this plateau and there will be a secret compartment open with the teleporter to the secret level.

You need the Red key which can be found through a teleporter behind the Bloodfall that is near Yellow key, Then inside Yellow door there is a lift up to a switch that will open the wall.
