How the hell do you get Parfait? What 5 stages?

Followed the guide, did the last four stages of the witch's castle and didn't have a ball go in a vortex in any. NOW the achievement won't unlock. I know the guide says five stages. Where's this fifth and how do I get the achievement to unlock? Do I have to play the entire game? run through the Swamp stages too? Someone, anyone, PM me and help!!! I want to get rid of this damned game already!!!
