I finally got this achievement after months of question. I shared the same problem a lot of people had with NEVER seeing the "break tackle" prompt even arise. Here's how I did it:

I had a team in Division 1. So, they were pretty tough by this point. Not sure how much it actually helps but I upgraded "strength" and "break tackle" primarily. I was experimenting with different plays, ie. passing, running, options, etc. I finally found the perfect play and I got the "break tackle" prompt 4 plays in a row. HERE'S THE PLAY YOU WANT:

On offense, obviously, scroll down twice while in your play book and choose, "REDZONE", which is the X button. Then pick the play called, "Rip Middle", which is the A button. This play will have you running straight up the middle and you're gonna want to initiate Clash (Left Trigger) as soon as the Running Back grabs the ball from your QB. Juke continuously by pressing the B button rather steadily...but not too fast cuz you're gonna be ripping the A button as fast as you can when you get the prompt.

My advice beyond using this play is that when juking in Clash you're gonna be moving in slow mo, take the time to try and aim your Running Back at oncoming defenders between jukes. 8/10 times they will wrap up with you and you will need to be tapping A as fast as you can. Like, veins popping out in your forearms and head, fast.

As well as this worked for me, it still took about 3 games to finally not get cheaped as you know the Blitz AI loves to do. But by the time i got it, I had my 3rd broken tackle in the first half. It still requires a small bit of luck but mainly making sure after a juke you're heading into oncoming traffic.
