It's been a long time since I've played, so I could be forgetting things, but you've basically got it already.
Luis exchanges the diamonds for Gracie. But in the ensuing firefight Niko and Packie find themselves in, the diamonds are dropped into the back of the truck. That homeless guy finds the diamonds in the trash (where this truck was heading), which can be heard on the radio news after completion (and I believe online too if you go to the news website).
