i have got every character up to level 9 and have NOT got the achievement for it why has this happened?

BEcause you have to beat every level with your character being level 9!

Another helpful tip is that in the level that you turn level 9 you do not have to redo that mission. You do not have to start a level at a level 9 only finish it as a level 9.
