I had the same problem. I finished mission 14 and the achievement didn't unlock. So I just carried on, it unlocked after completing mission 16.

The game has a strange notion of what counts as a "Mission" for the purposes of this achievement. Don't go by the "Story Missions Complete" tally in the menu if you're trying to get this achievement - instead, just concentrate on doing all of the missions and you will unlock all of the relevant achievements at one point or another.

I just completed all but 1 achievement (100 kills which I had from playing it for a bit a while back) in the last week. I had no issue with any achievement unlocking bugs. But noticed that sometimes the achievements for story missions wouldn't unlock when expected, sometimes it would happen after starting or finishing the next mission. Keep at it, I can confirm all the achievements work just fine.
