It seems a lot of people are having some confusion over the achievements for:

-Beating the game
-Beating the game on hard
-Beating the game in FPS mode
-Getting gold in all missions

So here you go, an easy 101 on this topic:

-Beating the game - Easy, complete every mission.
-Beating the game on hard - Easy (hard?), complete every mission, on Hard difficulty. Doesn't matter the order, you can replay any mission that you've already beaten.
-Beating the game in FPS mode - Beat the game to unlock FPS mode. Then replay every mission in FPS Mode/Difficulty. Doesn't matter the order, you can replay any mission that you've already beaten.
-Getting gold in all missions - To get gold in a mission you must
1) Make all responses (the A button responses)
2) Destroy every emitter
3) Listen/view every radio or TV broadcast*
4) Complete all optional missions (photograph 3 statues, destroy all meth labs, etc) and
5) Rank Perfect for every POSSIBLE forensics task in the level
Doesn't matter the order, you can replay any mission that you've already beaten.

So to summarize: you can obtain beat the game and all golds on one playthrough, you can obtain beat the game on hard and all golds on one playthrough, you can obtain beat the game in FPS mode and all golds on one playthrough. Or you can obtain all golds across several playthroughs.

You cannot complete Hard Mode and FPS Mode at the same time. You cannot complete FPS mode and beating the game at the same time (you must complete the entire game before FPS is available).

You can also obtain the achievements for beating one mission in FPS or Hard mode on ANY mission, it does not have to be the FIRST mission.

*You do not have to listen to the entirety of the broadcast for it to count

This information is true and correct. To further add to this, you can check your progress for all acheivements if you go on the main menu and go to Extras ---> Achievements. You can see which missions you've beaten on Hard/FPS how many of each type of chain attack you've done... etc.
