In the upper-right, just to the left of the HUD map, you'll see two vertical bars -- one is the wanted level with the Montano gang, the other the wanted level with the government authorities. The object is to get the wanted level of government authority bar (I don't know if this achievement will work with the Montano bar, alternatively) up to 5, and keep it there for 5 minutes -- which can only be done by CONTINUING to kill after getting it to level 5 (otherwise, over time, it will come back down to 4, even if you're not around one of your safehouses).
Personally, I did it by killing civilians on the starting island, near that eastern-most base (just running back and forth between the town and the base, shooting the cars first, to bring the civilians out), and, strangely enough, no police, troops, or helicopters ever showed up . . . whether that was due to location, or fluke, I'm not sure, but it made the task quite easy, as no one ever shot back at me the whole time.
One can bring the wanted level of either bar down most quickly by hanging around one of your safehouses -- you'll notice the wanted levels come down quicker than anywhere else.

The easiest way is to get the agency jet boat go near a miltary base and build up istant heat then up the rivers for five minutes and you have your achievment and to lose it go back to your safe house or let yourself die. As for heat the blue and white square next to you map is the government heat and it says what level your heat is on.

There are actually 2 "Wanted" bars. One is for the Montonos and the other is for for actual police. You get them after you by basically doing things that draw negative attention to yourself in their presence. Firing a gun at one of their cars is a sure fire way to do it. The Montonos are purple, and the police are... well... police-like.
