After completing the game, I was going to play a chapter again, but noticed the main menu still said continue, so I clicked that instead.
** SPOILER **
It loaded the end, where the daughter pushes the dude over the balcony.
I didn't fire a shot, just pressed A to skip the cut scene and up pops the "Old School" achievement.
