I'll give it a shot, it's pretty late where I live so I'll do some more tomorrow. Also, I dunno how to add pictures to it.

By completing the tutorial mission, you have proven yourself as an officer of Starfleet.
This achievement is gained after the first level, "Those in Need". It shouldn't give any real problems as it's just a tutorial. Just follow the instructions the game gives you and you should be fine.

You have completed all the missions in the Enterprise era.
This achievement isn't too hard to get, you'll unlock it upon completion of the fifth mission, "Stirring the Hive".
Admiral Kirk Award 50 http://www.xbox360achievements.org/forum/../images/site/gp-g.jpg
You have completed all the missions in the Original Series era.
Same as above, except it unlocks after the final mission of TOS, which is the tenth mission, called "Omega".

You have completed the single player campaign on Ensign difficulty.

Complete the single player campaign on "Admiral" difficulty.
Complete the final mission of the TNG era, and of the game, called "Logical Conclussions" on the respective setting to unlock this. I'm not sure if they fixed it but I remember hearing about a bug where if you changed the difficulty half way through the game (even lowering it) you wouldn't get any of the achievements. So BEWARE, just play the game on the same difficulty throughout just to be on the safe side. Difficulty can be ajusted through the options menu.

Complete the Enterprise era without losing a single ship.

Complete the Next Generation era without losing a single ship.
Complete the respective eras missions without getting a ship destroyed in any of them. If any get destroyed then restart the mission, unfortuantly there is no checkpoint system, so if you lose a ship at the very end then you have to do the whole mission over again.
If any of your ships take a beating, then warp them away to the edge of the map (which is usually free from hostiles, but check to make sure first), open up the LB menu, and repair everything, then press X and put all power to shields to get them recharged faster. If you do this for each ship when it gets into trouble you should be fine.
It's really getting quite late now, so I'll make a start on the individual mission achievements tomorrow after work.

Protect all the medical ships in "Be My Shepherd".
This is the third mission of the game, in the Enterprise Era. You need to save all of the medical ships. You should have the Enterprise, and the Coto. Buy a Yorktown battleship to make things easier. Open up the map using the BACK button, and see where each medical ship is going to. Send each ship along with each of the medical ones. Keep checking the map to make sure you're in the same area as the medical ship and not on the other side of the planet. Keep checking on the map to see where the Romulans are coming from, and take them out. The computer should let the other two ships deal with the other two enemies. When the medical ship has dropped off all the supplies follow it to the next planet and repeat. The mission isn't too hard, and if you do need to repeat the mission then at least you will know the order of where to travel to.

Keep the poisonous spore count low during "Poisoned Well".
This achievement is on the fourth mission, you need to keep the spore count below 1000. Each of the planets has a spore facility nearby, so you need to get rid of these as quickly as you can. I sent two ships around them clockwise and one counter-clockwise to speed things up. You should assign the ships to go to different planets while the Enterprise is following the first spore ship. When you're finally able to attack the stations then you should send the Enterprise to another planet, so a good idea might be to place all the ships a couple of planets apart, so that they each have a couple to do. Indivdually select the other two ships and attack the facilities. Ignore the Romulan ships, this needs to be done quickly. When it's destroyed then warp to the next planet along and repeat. Follow whatever route you like, but remember the Warp drive doesn't work when you're too close to the planet. Again, this shouldn't provide too much of a problem.

During "Stirring the Hive", knockout as many of the enemy fleet as you can in one blow...
The magic number here is 12 ships in one blast. Send the Enterprise to the top left one (not the top one) and wait. Put all power to shield (platted hulling) and open up the map, count how many ships are around you, and when you cound 12, hail the sensor relay. The achievement should pop up, you can then warp to the top most station, then the others downwards and back to the start.

Complete mission objectives in "The Squeeze" without the aid of starbases. You maverick renegade.

Secure all enemy strongholds in "The Squeeze".
The opposite of the Lone wolf of Makus and Pacifist of Makus, so you will need to complete the mission twice to get all the achievements. Go to each of the Klingon Starbases first, attack them until their shields go down, and warp a team across to capture it. When you've got them all you will get the achievement and can abort the mission to restart for the others.

Complete "The Squeeze" without spilling any blood…Ok, for this achievement you'll need to complete the squeeze without blowing up any Klingon ships. Send your other ships away to one of the corners closest to you at the start. Head down to the objective around the nebula. Put all power to weapons for now. Target the objective and open fire with phasers, when prompted put a tractor beam on it and put all power to engines. Now fly through the nebula, the Klingons shouldn't follow you through, and with full power to the engines you'll easily outrun any that do. Fly back to the spacestation and collect your two achievements.
Again, out of time for now, but I'll post some more tomorrow. I need to check some things for Behind Enemy Lines too about those achievements.

I did this by approaching the targets from the top. The AI seems to only think on a 2D scale so patrols just go around on a horizontal path.

Don't let the friendly AI ship get destroyed. At the start of the mission warp all of your ships to the friendly AI ship and save it from being destroyed.
Champion of the Fleet20 http://www.xbox360achievements.org/images/site/gp-g.jpgAll allies must survive in "At the Gates".
I took my toughest ship and went right out of range of the Federation turrets and started taking them out one by one. Do not use the Enterprise for this or the friendly AI ships will follow and get torn up. Once you clear the turrets then bring up the rest of the fleet and finish the mission.

I kept the large battleship in my fleet from the Enterprise Era. Just start the Next Generation mission with one of the Enterprise Era ships in your fleet and you get the achievement. Then you can abort the mission and sell that hunk of junk.

Not sure how I got this but it was during the last TOS Era mission. There were a bunch of small ships and I kept lauching torpedos when I could.

I also got Hero of Kathra in the same run here it goes:
-First deploy all energy to weapons and then go with all ships to help the friendly AI ship and destroy all that's in that sector, I took the repair station down as starters then the minning fac. and then a the few ships.
-Then go to the right on the map, and destroy the 3 repair stations first, and then the remaining ships at that location.
-Final blow:Go to the last location and before taking out that last weapon of the klingons, first destroy the ships in that area, then the turrets,(i warped out of danger a couple off times, so you can repair the ships), and finally when there are no more targets left, destroy the super weapon.

You only need the enterprise, so fly the rest off the fleet to the bottom off the map.
-First Objective is to Investigate the Federation beacon.
This is really easy, you find it and it just gives you some info about the situation.(don't destroy any enemy ships or turrets)
-2nd Objective is to Destroy enough warp dampeners to re-enable warp, this takes some time, i just destroyed the bottom and left row of dampeners(watch out for patrolling ships), dont destroy any ships or turrets only dampeners, till you destroyed enough.(if you are being chased by enemy ships, just put all power to engines, and fly away,enemy ship will resume patrol in time)
-3rd Objective is to Ascertain the location of the USS Defiant and her crew.
I will just tell you where they are. They are at the Romulan outpost sitting in the middle of the map.(Just warp to the outpost, and again don't destroy any enemy ships or turrets)
-4th Objective is to lower the base's shields and beam an away team into the base to accomplish this objective.
Put all power to weapons and take the shield down, again don't destroy any enemy ships or turrets)

Simply replay the mission - use the cloak and come in from above/below the targets to be scanned leaving the weapons platform untill last.

So about how long is the campaign in the game cause i just started playing it and since the achievements are not stackable, i have to do it three times, so i would like to know how long it takes.

It depends on if you rush through or actually play to get all secondary objectives. The admiral level will take the longest I would estimate about 6 hours depending on your skill level.
You can get 90% of all achievements on ensign and if you boost for the online that should only take about 20 mins.

I tried to get the scourge of kathra acheivement, and followed the instructions given in this review and the achievement did not unlock, anyone still playing the game that can give me some tips?
