Starting off, you will need patience, and somewhat higher than novice skill.
This is not a hard achievement, just difficult due to a perfect run, and zero information on how to obtain this pesky achievement.

Set the game mode to Xbox360 Mode, and Difficulty to Original, using only 1P single.

Here are a few tips to act as guide lines going for this achievement:

Vulcan - Red power up, which is the "machine gun" and stock firepower when you start the game. DO NOT pick up power ups of any other color, besides the Red Vulcan power up. Picking up purple or blue will result in losing out on this achievement, and will need you to cut your losses, and restart on level 1-1.

Secondary weapons - The Secondary weapons "H", "M", "P", "R" can be obtained however you see fit, or are comfortable with. These do not prevent the achievement from popping. I prefer "H" as they act like a homing missile towards targets which are not directly in front of you which is helpful when trying to concentrate on when to obtain the Red vulcan powerup.

Bombs - Bombs are OK to use during your playthrough. Use the bombs Strategically when needing to obtain your powerup, and use them sporadically. Be sure as a helpful tip to beat the game multiple times to obtain the highest max bombs you can set in your options menu, along with lives.

Spawning After Death - Your ship will spawn on the left side of the screen, near the bottom. When you are playing, try to keep your ship more so on the right side of the screen, so when/if you die, your items/power ups will be towards the center of the screen, while your ship spawns in left side, giving you a few seconds to re-adjust your situation.

The achievement will pop on level 1-7 after you destroy the final boss, when your stats are being displayed.

While shooting, the power ups will change colors, but when you stop shooting, the closest powerup will act like a homing missile towards your ship. Be smart about this, sometimes resulting in a purple hiding behind a Red, or a secondary will hone in on you, resulting in the Red to turn to blue, and so on..

If you need to Die for bombs sake, try to die in less populated areas of the level so you can gain footing again before a final boss. Everytime you Die, 2-4 power ups appear in the center of the screen, in which if you do not collect the red power ups in time, and die again, 2-4 more power ups will appear. On the level 1-6 and 1-7 is when you will need to worry about having 9+ power ups on screen at a time, in which the "tornado of power ups" will gradually get larger and larger, which is NOT what you want.

The wording for this achievement is difficult to understand WHAT specifics you need, but hopefully these few tips will help out anyone with questions about this achievement. Any questions are appreciated.
