How to start New Game + (Spoilers Allowed)

So I am close to the end of the game and was wondering how exactly to initate the new game plus. I know its not related to the main menu or anything but I can't really find anything on how to do it.

Also what exactly carries over?

Vocation ranks?
Storage?
Pawn & Player customization?

Sorry if somebody has already posted on how to do this but I haven't seen it. I know there has been some discussion but not a step by step that I have seen.

I go over it in the pseudo achievement guide I wrote. Everything carries over. You need to stab yourself with godsbane and it will take you to ng+ right away
