Would I then be able to play the DLC on my real account? Or would it only work on the Japanese account? I don't want to buy it for a different account and then not be able get the achievements on my real account.

Onces its downloaded on the HD you will be able to play it. I was able to get Earthworm Jim free when I created a JP account (when it was a promo)

seems to be 8 additional missions total right? considering missions in original game was very very easy, it will take no more than 1hour
