Arcade Mode: How do you "insert a coin"?

When i try to beat arcade mode and get killed the option of "inserting a coin" doesnt seem to appear. Where is it supposed to happen ? Plus, last time i tried, after getting killed, i couldnt get in the leaderboards because the game said that i wasnt logged into xbox live even though i actually was! Is it some kind of bug in the game and could it be why i cant "insert a coin"? I really would like to get some answers on all of this.

In arcade mode you dont get continues unless you have 2 controllers
