OK I've done
Ryloth - 2, 7, 9, 11
Juma 9 - 2, 5, 10
Bephor - 1, 6,

as Cad Bane but the damn thing won't unlock. I've done these levels at least 3 times! what is going on?

Because you're missing one. If you're going off the descriptions for the missions, you'll miss the same one I did. Here's the list:

Ryloth:
Missions 3, 7, 9, and 11.

Juma-9:
Missions 2, 5, 8, and 10.

Behpour:
Missions 1 and 6.

You're missing Juma-9 mission 8. You start as Clones and then move on as Mace and Kit.
