Medal points set?

Does anyone know if the points needed, the survival time needed and the time spent on the game is set before you play a game for the first time?

Eg. If you're an ex addict of the game and can complete it on your first attempt will you instantly get gold for all 3 medals?

yeah the time to beat is the same for everyone. check the achievement guide and there is a spreadsheet that tells what you need for every medal
