When you finally catch up with Serial Killer X at the end of the game, you have the choice to let him live (Compassion) or kill him (Revenge) for the respective achievement. Whichever you choose, simply reload your last save to find yourself back in the last boss battle, pick the opposite to whatever you did previously and there have it - Both achievements.

This badge is earned for finding the tv set in the first Chapter. When the cop throws the axe to you from the opposite building, backtrack until you find a door that requires an axe to get through. Tv set is in this room.

When you find the elevator with Serial Killer X's fingerprint on Chapter 2, turn right. The TV is in the room that you need the axe to break through. Activate the Tv for the achievement.

After Killing the three crawling psychos in Chapter 4, you'll see a red metal box in the corner. Behind this, there's some plywood in the way. Move it to find the TV.

During Chapter 6, after you find Bird #3 under the plywood, head down the left hall and you'll find the TV inside the small room there.

Awarded for completing one level (Bronze), three levels (Silver) and the entire game (Gold) only using melee weapons. It's not tough as it seems - simply use your tazer to get the jump on any distant enemies with firearms. Also know that you can pick up firearms and even use them as melee weapons. Just make sure you don't fire a single shot with them!

.......this achievement , which is awarded for finding evey weapon in the game. You don't need to kill anyone with them, just make contact. Here's an exhaustive list of weapons that you can tick off as you find them: 2x4 Bolts, 2x4 Burning, 2x4 Burnt, 2x4 Nails, Cleaver, Clothes Rack, Colt .45, Conduit, Crowbar, Desk Drawer, Desktop, Fire Axe, Fireplace Poker, Gas Pipe, Handrail, Large Pipe, Large Plank, Locker Door, Mannequin Arm, Paper Cutter, Pump Shotgun, Rebar Bent, Rebar Concrete, Rebar, Revolver, Rifle, Sawed Off Shotgun, Shovel, Sign (Subway), Sledgehammer, Small Pipe, Steam Pipe, Stick, Sub-machinegun.
