I don't know about you guys but the first time I reached General rank, I did not unlock the achievement.
If you are having trouble, the only solution I've found is by starting a whole new game. Yes, your rank will reset, but it's not a big deal. You can rank all the way back up in as less as an hour.
To rank up fastest
-Complete the bonus objectives
-Try and finish under the target time
-Simply by completing levels and main objectives
By the time you reach the next to last level, you should be a General.
