I've been hacking and slashing for well over about 1hr and I still only have the first quest bubble complete. Anyone know why? I'm playing on normal and gee wizz!

You can only get one level per chapter play. Finish the chapter and replay it to get level two, repeat for quest level 3.

It's possible (and very easy) to play this entire game on easy and not unlock a single achievement. The quest achievements are all actions that have to be done 3 separate times to unlock, there are achievements for finishing the game on any difficulty (other than easy), and there are 3 for unlocking all the bonus clothes for the girls (all of which you get through doing quests and leveling to 30).
