This is NOT a list of all the flags and templars in the game. It is a guide as to how to play your game to avoid getting the problem of having one more flag or templar to finish.
First of all, don't walk through the game and say things like, "Hey look, a flag! I'll just wander over and get it!" Doing that will mean that you lose track of which flags you've got and which you haven't. I personally had this problem. I was trying to get all the flags, using a map. Whenever I couldn't find the flag I never knew whether it was because I already had it, or because I just couldn't see it. Templars are more difficult, because they'll attack you, but you should still try and run. If you have to kill him, then make a note of where he was.
So what you should do it play the game, and avoid ALL the flags and the templars.
Once you are about to finish the game, and you have unlocked all the areas in all the cities, NOW you go back and get the flags. As for which flag/templar guides to use, I strongly reccomend the one from GamesRadar.
It provides highly detailed maps of each area, marks all the flags with numbers, and the templars with letters. It then has small amounts of text describing exactly where each and every flag and templar is, along with tips on how to get to the difficult places.

Finally, if you're thinking, "Well that's all very well, but I've already got some of the flags while playing through", then the easiest solution to this is, quite simply, to restart your game. Chances are you missed the Conversationalist achievement, and possibly Hungerer of Knowledge aswell, so you can get those on your second play through.
Trying to find the ones you've missed is pointless and stupid. It's MUCH faster to just restart.
